Employers use this Checklist when you are ready to Enroll:

1. Complete the Data Sheet
   
   
2. Complete the Authorization Form with a Voided Check
   
   
3. Complete an Enrollment Form for each person to be insured (Employer and Employee), calculate the total premium from the plan selected and write a check for deposit premium.
   
   
4. Mail deposit premium and all enrollment materials to:
